Holotropic Breathwork: Why 30 Minutes Curls Your Hands

Most people who breathe this way feel their hands lock. A 2025 study measured the carbon dioxide drop that causes it, and what else that drop opens.

Holotropic breathwork is prolonged voluntary hyperventilation, usually practised in sessions of thirty minutes or longer. Most people who do it feel their hands tingle, then curl inward and lock. The name for that is tetany. It happens because fast breathing strips carbon dioxide out of the blood faster than the body makes it, and it reverses within minutes once the breathing slows.

Why breathwork gives you claw hands

Carbon dioxide is not waste. Dissolved in blood it forms carbonic acid, and that acid is most of what holds your pH where it belongs. Breathe hard for long enough and you blow the acid off. Blood pH climbs. The condition has a name, respiratory alkalosis, and it is the whole mechanism.

Calcium is what turns the rise in pH into a spasm. Roughly half the calcium in your blood travels bound to albumin, and only the free half, the ionised fraction, does the work of settling nerve membranes. Albumin binds calcium more tightly as pH rises. So the alkalosis does not remove any calcium from you. It parks a larger share of it on a protein, and the free share drops.

Nerves with less free calcium around them fire at a lower threshold. They start firing without being asked. The small muscles of the hand contract and stay contracted, the thumb draws in across the palm, the fingers bunch, and you get the shape people call claw hands or lobster hands. The feet can do the same thing. Tingling around the mouth arrives first for most people, because those nerves are close to the surface.

A 2023 case report from an emergency department in Orlando shows why this confuses people. A man arrived cramping, with tetany and visible vasospasm. His total serum calcium came back high. His ionised calcium came back low. Routine bloodwork reports the total, so the number that matters was the one nobody would have ordered.

If your hands start to curl

  1. Stop the fast breathing. Let the next breath happen without steering it.
  2. Close the mouth and breathe through the nose only, which halves how much air moves.
  3. Make the exhale longer than the inhale, and let a pause sit at the end of it.
  4. Rest your hands in your lap and leave them alone. Stretching a spasming hand makes it grip harder.
  5. Wait. Ordinary breathing returns the chemistry in a few minutes, and the tingling fades after it.

Tetany in breathwork is not emotional release

The breathwork world has its own account of this. Tetany gets described as trapped emotion leaving the body, as armour breaking, as energy moving through a blockage. Facilitators will tell you the tightest hands belong to the most defended people.

That account is not supported by anything measurable, and it has a practical cost: it tells a person whose hands have locked that the correct response is to keep going. The physiology says the opposite. The spasm is a readout of how far your carbon dioxide has fallen, and the way to resolve it is to let the carbon dioxide come back.

None of which means the experience is meaningless. People do surface real material in these sessions. The claim worth dropping is the narrower one, that the hand spasm itself is emotional in origin. It is chemical, it is well described, and it happens to anyone who moves that much air, defended or not.

What holotropic breathwork does to the brain

The same fall in carbon dioxide that curls your hands is doing something upstream.

Hypocapnia constricts cerebral arteries. Less blood reaches the cortex, and the effect is quick and measurable. One study tracked blood flow velocity falling during nothing more dramatic than mental arithmetic that made people breathe faster than they realised.

In April 2025 a team from the MIND Foundation, Charité and Imperial College published the study this practice had been waiting for. They tracked physiology and experience across full sessions, comparing Holotropic and Conscious-Connected breathwork, and found that the drop in end-tidal carbon dioxide correlated with the onset of the altered state at r = -0.46, p < 0.001. On standard questionnaires the states resembled those produced by psychedelics across several domains, ego dissolution among them, and the depth of the state predicted how people felt afterwards. The two styles of breathwork produced almost identical outcomes, which suggests the method matters less than the chemistry it drives.

Stanislav Grof developed holotropic breathwork in the 1970s as a legal substitute for psychedelic therapy after LSD research closed down. A 2007 paper reframed it in plainer language as a prolonged voluntary hyperventilation procedure, and proposed that its therapeutic value, if it has one, comes from repeatedly extinguishing avoidance rather than from anything mystical. That paper was explicit about being a hypothesis. Nineteen years later it still mostly is.

The evidence base is smaller than the practice

Eight studies on holotropic breathwork are indexed in PubMed. Eight, for a practice with a fifty-year history, a global network of certified facilitators, and a commercial descendant industry selling sessions by the hour.

The 2023 review in Neuroscience and Biobehavioral Reviews put the whole family of these practices under one heading, high ventilation breathwork, and concluded that the effects are real and the mechanisms are largely unmapped. It reads as an invitation to run trials rather than a summary of them.

So the honest position sits between the two loud ones. The physiology of what happens to your hands is settled. What a session does for depression or trauma over months is not, and anyone quoting you a percentage is quoting something that has not been measured. The same fast-breathing chemistry runs through kapalabhati, where the altered states are older and better documented.

Is holotropic breathwork safe, and who should not over-breathe

For a healthy adult in a supervised session, tetany is uncomfortable and self-limiting. The risk sits elsewhere.

Sustained hyperventilation raises intraocular pressure, lowers the seizure threshold, and puts a load on the heart. The standard exclusions across teaching bodies are consistent: cardiovascular disease, uncontrolled high blood pressure, a history of seizures, glaucoma or retinal detachment, pregnancy, recent surgery, and a personal or family history of psychosis. A 2025 case report described a child with hyperventilation syndrome presenting with electrolyte disturbance and cardiac involvement, which is a reminder that the chemistry is not always trivial.

Breath-holding after over-breathing deserves its own warning. Blowing off carbon dioxide removes the urge to breathe without adding oxygen, which is the mechanism behind shallow-water blackout. Never do this in or near water.

If your interest in this is anxiety rather than altered states, the direction of travel is the reverse one. Panic and carbon dioxide have a relationship worth understanding before you go looking for more of it.

What pranayama measured that over-breathing does not

The word gets translated as breath control, which loses the useful half. Prana is breath. Ayama means restraint, or extension under measure. The classical practices counted. A breath was set against a matra, a unit of time, and the ratio between inhale, retention and exhale was fixed and then lengthened by degrees across years. Kumbhaka, the held pause, exists to make the breath smaller and slower rather than larger and faster.

That tradition also recorded what happened when people got it wrong. The Hatha Yoga Pradipika says that pranayama performed properly eradicates disease and that improper practice generates it, and then lists the symptoms: hiccough, asthma, cough, pain in the head, the ears and the eyes. Those are the complaints of someone who has been moving too much air. The text is a practice manual, not scripture, and the passage reads as what it is, a note about a failure mode observed often enough to be worth writing down.

The verse before it is the one worth keeping. Lions, elephants and tigers get tamed by degrees, and breath is tamed the same way, and hurrying kills the person doing the hurrying.

The modern high-ventilation practices invert that instruction on purpose. They are looking for the threshold, and they cross it deliberately. That is a legitimate thing to want, and it is a different project from the one the word pranayama describes. Knowing which one you are doing tells you what your curled hands mean, and it tells you whether to keep breathing or to stop.

Why do my hands cramp up during breathwork?

Fast breathing removes carbon dioxide faster than your body produces it. Blood pH rises, more calcium binds to albumin, and the free calcium your nerves depend on falls. Nerves fire at a lower threshold, and the small muscles of the hand contract without being asked. Slowing the breath reverses it.

What causes tetany in breathwork?

Low carbon dioxide, not low calcium intake. The total calcium in your blood does not change during a session. What changes is how much of it is free rather than bound, and that fraction is governed by pH. A 2023 case report recorded exactly this pattern: ionised calcium low, total serum calcium high.

How long does the tingling last after breathwork?

The spasm usually releases within a few minutes of breathing normally. Tingling in the fingers and around the mouth can linger longer, sometimes a quarter of an hour, because the blood returns to its resting pH more slowly than the muscles let go.

Is holotropic breathwork safe?

For most healthy adults a supervised session is tolerated, but the practice has real contraindications, and the published research is thin enough that nobody can quote you a reliable risk figure. Cardiovascular disease, uncontrolled high blood pressure, a seizure history, glaucoma, pregnancy and recent surgery are the standard exclusions. It is not a practice to try alone from a video.

Does tetany mean the breathwork is working?

It means you have moved a lot of air. Tetany tracks how far your carbon dioxide has fallen, and nothing else. Plenty of people reach a deep state without their hands curling at all, and plenty of people get the spasm and feel nothing else worth reporting.
