CO2 Tolerance Test: What Kumbhaka Was Actually Training
A breath-hold timer scores how loudly your body objects to carbon dioxide. The classical retention ratios were counting something else entirely.

The CO2 tolerance test measures how long you can hold your breath after an ordinary exhale, up to the first clear urge to breathe. Most people meet it as the BOLT score, a number to raise. What it tracks is how strongly your body reacts to rising carbon dioxide. That reading is real, and it has almost nothing to do with how fit you are.
The confusion is understandable. A stopwatch, a rising count, a target of forty seconds: everything about the format says athletic test. Sit with what the clock is actually watching and the picture changes.
What the CO2 tolerance test actually measures
Hold your breath for twenty seconds and your blood oxygen barely moves. Carbon dioxide, on the other hand, climbs from the first second, because your cells never stop making it. So the timer is not counting down a reserve of oxygen. It is counting up a signal.
And the signal is stranger than the popular account suggests. In a 2006 review that remains the clearest account of what stops a breath-hold, the physiologist M. J. Parkes established that you cannot switch off your respiratory rhythm at all. It runs the whole time. You suppress its expression and hold the chest where you put it, while the rhythm keeps firing underneath. The breakpoint arrives when suppression loses.
What produces that loss is not simply blood chemistry. Parkes found the breakpoint could not be pinned on lung volume, on arterial gas pressures, or on the carotid chemoreceptors alone. A likelier culprit is the diaphragm itself: held in an unusually long contraction, it becomes under-perfused, and its own muscle chemoreceptors start reporting distress. The urge you feel is closer to a cramping muscle than to suffocation.
So the number on the stopwatch describes a negotiation. On one side, an involuntary rhythm and a complaining diaphragm. On the other, your willingness to sit still while both get louder. That is worth knowing about yourself. It is a different fact from the one the test is usually sold as.
One measured hold
- Sit. Breathe normally through the nose for four or five cycles until nothing feels forced.
- After an ordinary exhale, pinch the nose closed. Do not fill the lungs first.
- Wait for the first clear signal to breathe. Note it without acting on it for two seconds.
- Release and let the breath return on its own. Do not gasp or over-breathe to catch up.
- Ask one question instead of checking the clock: where did attention go while you waited?
BOLT score vs CO2 tolerance test: the same hold, two claims
The two names get used interchangeably, which muddies things further. The BOLT protocol is specific: breathe in normally through the nose, breathe out normally, pinch the nose, and stop the clock at the first distinct desire to breathe. Its designers are explicit that this is not a maximum hold. Most versions of the broader "CO2 tolerance test" ask for something else, often a controlled exhale counted to its end, or a hold pushed toward the limit. Same body, different instruction, incomparable numbers.
The BOLT claim is that the resulting score reflects reflex control of breathing and therefore predicts exercise tolerance. In 2024, researchers at the Institute of Sport in Warsaw tested it. They took 49 highly-trained speedskaters and ran the BOLT against a Wingate anaerobic test measuring peak power, total work and power drop, and against a cardiopulmonary exercise test measuring maximum oxygen uptake and time to exhaustion.
Nothing lined up. Across every performance measure, BOLT scores carried no predictive relationship. In the authors' careful phrasing, BOLT may not be a useful proxy for fitness in highly-trained individuals.
Read that as a boundary rather than a demolition. Athletes are a narrow population, and a test can fail there while describing something true elsewhere. What the study removes is the specific promise: that a longer comfortable hold means a better-conditioned body.

Breath hold time and what it predicts
Here the evidence gets more interesting, and considerably more careful.
Sensitivity to carbon dioxide is a genuine individual trait, and psychiatry has taken it seriously for three decades. Donald Klein's suffocation-alarm hypothesis proposed that panic disorder involves a mismeasuring alarm system, one that reads ordinary carbon dioxide as imminent asphyxia and fires a full-body emergency. The 35% CO2 inhalation challenge, still a standard laboratory model, provokes panic-like responses in susceptible people with unusual reliability.
If that is right, a very short comfortable hold might say something about how your nervous system interprets its own chemistry. The literature does not settle it. Later chemoreflex work comparing panic patients with healthy volunteers found no difference in central or peripheral chemoreflex sensitivity, which suggests that whatever the alarm is, it may not live in the reflexes the breath-hold recruits.
Two honest conclusions survive. Your breath-hold time reports on a real dimension of your physiology. And nobody has yet shown that the number diagnoses anything, in either direction, for a person sitting at home with a phone timer.

How to improve your BOLT score
The score does move, which is part of its appeal. Weeks of slow, quiet nasal breathing raise it for most people, largely by lowering habitual ventilation so that carbon dioxide stops being flushed out all day. Repeated maximal holds also raise the number. They raise something else too. Pushing breath-holds as a performance is how people arrive at the tingling hands and cramping fingers that follow hard over-breathing, and the practice never asked for that. The classical texts hedge retention with a teacher for the same reason, several centuries before anyone could explain the chemistry.
There is also a quieter cost. A number that moves invites daily checking, and daily checking turns a practice into a scoreboard. You can feel the shift happen. The sit stops being the point and becomes the thing you do in order to have a better sit tomorrow.
What kumbhaka ratios were training instead
Which brings us to the part the tradition would recognise.
Kumbhaka, retention, is measured in the classical sources with the same apparent precision a stopwatch offers. The Hatha texts give a ratio of 1:4:2 for puraka, kumbhaka and rechaka: inhale for one count, retain for four, exhale for two. The counts are matra, roughly a second each. Three grades are named, twelve matra for the lowest, twenty-four for the middle, thirty-two for the highest. Written out like that, it reads like a leaderboard with three tiers.
The stated purpose sits somewhere else entirely. The Hatha Yoga Pradipika explains its interest in the breath in a single line: when the breath moves, the mind moves; when the breath is still, the mind becomes still. Retention is instrumental. The lungs are a handle on attention.
Patanjali is more specific still. Having described pranayama across four terse sutras, he names the fruit of it in a fifth: dharanasu ca yogyata manasah, the mind becomes fit for concentration. Not capacity. Not endurance. Fitness for the next limb, which is holding attention on one thing.
That reframes the ratio completely. Four counts of retention hands you a stretch of time in which nothing happens except a signal getting louder and a decision not to act on it. Your attention has nowhere to be. It is a concentration exercise wearing the clothes of a breathing exercise, and the count exists to make the stretch long enough to be difficult.
The modern number and the classical ratio look alike on paper. They point in opposite directions. One asks how long you lasted. The other asks where you were while it happened.
