Sitali Pranayama: The Cooling Breath and How It Works

Sitali pranayama is the yogic cooling breath: you curl the tongue into a tube, draw air in through it, and let it out through the nose. What Sitali pranayama is, and how to do it
The practice is unusual in one respect. It is the only classical breath family drawn in through the mouth. Every other pranayama, from ujjayi to nadi shodhana, insists on the nose, and the texts are strict about it. Sitali breaks the rule on purpose.
You roll the sides of the tongue upward until it forms a channel, let the tip rest just past the lips, and draw air slowly through it. Then the mouth closes and the breath leaves through both nostrils. The name comes from śītala, cool, and the sensation arrives immediately. Air moving across a wet tongue evaporates moisture from its surface, and evaporation carries heat away with it. The tongue cools. The soft palate cools. The throat feels like you have swallowed cold water without drinking anything.
The vividness of that is not imagination. Cold sensation does not arrive from a thermometer wired to the brain; it comes from receptors, and the ones lining the mouth and nose are unusually dense and unusually quick. The same receptor family that reports cold also fires in response to menthol, which is why a mint feels cold in a room that has not changed temperature. Drawing air over an exposed, wet tongue stimulates that machinery directly. You are feeling something genuine, reported by the most responsive sensory surface you own.
Eight breaths over the tongue
- Sit upright, shoulders loose, jaw unclenched.
- Roll the sides of your tongue upward into a tube and let the tip sit just past your lips. If your tongue will not roll, press the tip behind your upper teeth and part your lips instead.
- Draw air in slowly through that channel over a count of about four.
- Close the mouth. Pause briefly if it is comfortable.
- Breathe out through both nostrils, slower than you came in.
- Repeat for eight rounds, then sit for a moment before you stand.
What the cooling breath does to blood pressure and heart rate
This is where the evidence is strongest, and it is unusually good evidence for a single named pranayama.
A randomised controlled trial published in 2020 assigned 100 patients with primary hypertension either to three months of daily sheetali or to usual care. The practice group showed significant reductions in blood pressure and a clear shift toward parasympathetic dominance across both time-domain and frequency-domain heart-rate-variability measures. The authors concluded it could be practised alongside regular medication in the management of hypertension. A separate randomised study in hypertensive patients found the same practice improved cognitive performance.
A 2025 comparison put shitali, sitkari and ujjayi head to head with simultaneous HRV and EEG recording. Sitali and sitkari produced the largest parasympathetic increases of the three, measured by RMSSD and pNN50, with ujjayi trailing both. For a breath usually filed under "something to do in summer," outperforming the best-known calming pranayama on the standard vagal indices is a stronger result than the tradition ever claimed for it.
The mechanism is not mysterious, and it is not unique to this breath. Slow breathing with an exhale longer than the inhale drives vagal outflow through the baroreflex: blood pressure oscillates with each breath, the exhale phase lets heart rate fall, and holding a slow enough rhythm brings those two oscillations into step. That is the same machinery behind slow-paced breathing generally. What sitali adds is a narrow oral aperture that slows the inhale for you without any counting, and a sensory event vivid enough to hold attention on the breath rather than on the argument you had at lunch.
The effects also compound. A six-month randomised trial in post-COVID patients found sheetali raised forced vital capacity from 3.3 to 3.7 litres alongside improved HRV, and the gains at six months ran well ahead of the gains at three. Whatever the practice is doing, it does more of it the longer you keep it.

Why the tradition called it cooling
The word doing the work in the classical sources is not thermometric, and reading it that way sells the tradition short.
The Haṭha Yoga Pradīpikā places sitali among the eight kumbhakas and assigns it a list of effects that reads oddly to a modern eye: fever, disorders of bile, enlarged spleen, hunger, thirst. Those belong to a humoral system in which pitta names a functional category covering heat, acidity, irritability and inflammation together. To call a practice cooling inside that vocabulary is to say it settles an agitated state, which is a claim about arousal rather than about degrees.
The instruments bear that reading out. When a team at the Patanjali Research Foundation recorded body temperature and metabolic gas exchange through sessions of sheetali and sitkari in 2020, temperature did not fall; it held and rose slightly, along with oxygen consumption. Drawing air through a narrow oral aperture is muscular work, and muscular work makes heat. That finding sits awkwardly against the English word cooling and not at all against what the texts were describing. The practice settles you. It was never a thermometer.

Sitali vs Sitkari: two versions of the same idea
Sitkari solves a problem the tradition noticed early. Roughly a third of people cannot roll the tongue, so instead of rolling it you press the tip behind the upper teeth, part the lips slightly, and draw air across the teeth with a soft hiss. The name imitates the sound. The exhalation still leaves through the nose.
The Pradīpikā describes sitkari at II.54 and sitali at II.57, treating them as a pair with overlapping effects, and the modern work agrees. The 2025 HRV comparison found both outperforming ujjayi, and the 2020 physiological work found no meaningful split between them. Pick whichever your mouth can do and stop worrying about the difference.
If the mouth-breathing element bothers you, humming bee breath reaches similar parasympathetic ground through the nose, and right-nostril breathing runs the other direction when you want alertness instead of calm.
How to practise Sitali pranayama, and who should not
Eight to twelve breaths is enough to feel the shift. That is a different scale from the one the trials used: the blood-pressure results came from ten to fifteen minutes daily, sustained across months. Both are worth knowing. The short version changes your afternoon, the long version changes your readings.
Timing matters more than duration. The practice is most useful at the points in a day when arousal has climbed and there is nothing to do with it: the flat stretch of a hot afternoon, the twenty minutes after a difficult conversation, the gap between finishing work and being any use to the people at home. Classical sources place it in the warm part of the day, which lines up. The one time to be careful is immediately before you need to be sharp, since a strong parasympathetic shift is the opposite of what a presentation wants.
The practice carries its own contraindication in its mechanism. Pulling unfiltered, unwarmed air across an open mouth is a poor idea if you have asthma or a reactive airway, a respiratory infection, or if you are sitting outside in the cold. Anyone with low blood pressure should treat the parasympathetic effect as genuine and stand up slowly afterwards. If you are managing hypertension with medication, this sits alongside an established pranayama protocol rather than replacing anything.
What you are reaching for in the middle of August is rarely a lower core temperature. It is the feeling of having stopped. Eight breaths over a curled tongue will give you that in about two minutes, and three months of them will show up in numbers a doctor can read.
